These reports have been provided to News19 by the Department of Social Services. Our request was to be given documentation on every Supervisory Visit and every Complaint Visit in Richland, Lexington, Saluda, Fairfield, Kershaw, Sumter, Orangeburg, Newberry, Calhoun, Lee and Clarendon Counties concerning licensed day cares for 2006. Some records could be incomplete, based on what we were provided. Some of the deficiencies recorded may have been fixed that day, or soon after the original inspection. We have also included all complaints provided, whether the determination was founded, unfounded, or unclear.

· Are you allowed to make surprise visits?
· Is the play equipment sturdy and safe?
· Are there enough caregivers?
· What is the discipline policy?
· Have you checked the day care’s record with D.S.S. for past problems?
